Patient being supported by family

Integrated Care

Integrated health care is a patient-centered care model that focuses on meeting the patient where they are in life across multiple facets. It is an approach to healthcare that is characterized by a high degree of collaboration and communication among health professionals to ensure all aspects of a patient's health has been addressed.
Integrated care brings together the different providers involved in the patient’s care so that, from the patients perspective, all of their services delivered are consistent and coordinated. Oftentimes, providers focus on single episodes rather than the patient's overall well-being. At Alium Health, we strive to provide the best integrated women’s healthcare across all areas of care.

Five core functions of integrated healthcare:

Comprehensive Care

Meeting a patient's physical and mental health care needs, including prevention wellness, acute and chronic care.

Patient Centered

Relationship-based care with focus on the whole person. People have unique needs, cultures, and circumstances.

We embrace uniqueness.

Coordinated Care

Clear communication across all areas of care from the patient to their care team, and every other Alium team member.

Accessable Care

Shorter wait times for urgent needs, with in-person and telehealth visits available.

Quality and Safety

We are committed to quality care and are always self-examining to improve.
